En Espanol Plan de Alergia Alimentarias

Por su estudiante cuando tiene alergia alimentaria.  Por favor completar y devolver a la clinica del escuela. 

Descriptive form♥ used to alert KISD to your child's food allergies. 

♥Please be aware, an additional form is required by KISD's Nutrition Department. It must be signed by parent and physician in order to provide substitutions and, alert the Nutrition Department staff to your child's food allergies. 

Most physicians provide their own form(s) for Asthma Action Plans.

The School Nurse can provide you with a form if your physician does not utilize a preferred form. 

Form required for all medications: creams, ointments, eye drops, ear drops, tablets, capsules, liquids, cough drops, balms, injectables and others. 

All substances requiring this form must be dropped off in the front office or directly to the school nurse by an adult.

Do not pack medications in your child's lunch box, purse, backpack, etc... or send them on the school bus.   This is a district safety policy.  Thank you for keeping our kids safe.

Complete this form if your student has had the varicella or chicken pox illness in the past and is exempt from varicella immunizations.
